The three involved in the head on collision on SR-260 at MP319 have been identified.

Show Low, Ariz. — Authorities have identified the three people killed in a head-on collision Sunday night on State Route 260 near milepost 319 outside of Heber-Overgaard.

The victims include two Timber Mesa Fire and Medical District firefighters — Brenna Kulikowski, 31, and Damon Thompson, 21 — along with Angela Marie Bouck, 46, of Pinedale, the driver of the other vehicle involved.

According to the Arizona Department of Public Safety, Bouck was traveling westbound when her vehicle crossed the center line into the eastbound lane and collided head-on with a Timber Mesa ambulance for unknown reasons. The ambulance was returning to Show Low after completing an interfacility transfer to a hospital in the Valley. All three died at the scene.

Bouk was listed as a missing person on Sept. 15 by the Navajo County Sheriff’s Office. The missing flyer stated she left her home on Sept. 14 around 11:00 to 11:30 p.m. and was determined to be missing. She was identified as the third person involved in the collision, which was made known to her family and the public on Wednesday afternoon, Sept. 17. She went unidentified for several days until investigators put the pieces together and determined she wasn’t missing; she was dead, and the silver vehicle she was driving was involved in the deadly collision on Sept. 14 at 10:19 p.m.

Timber Mesa Fire and Medical District Chief Randy Chevalier said the collision was among the worst he has seen in his career.

“Head-on collision, extreme damage. And I’ve been on a lot of accidents over my career. And it was, it was horrific,” Chevalier said.

He remembered the fallen firefighters for their dedication and compassion.

“Brenna was certainly top-notch and such a compassionate person and a fantastic paramedic,” Chevalier said. “Damon’s parents said Damon always wanted to be a firefighter, and he was a dang good firefighter.”

Governor Katie Hobbs also released a statement offering condolences:

“I’m heartbroken to learn two brave firefighters lost their lives in a tragic crash near Show Low. These heroes dedicated themselves to protecting our communities, and my thoughts are with their families, loved ones, and fellow first responders during this difficult time.”

Procession to Show Low

On Wednesday, the bodies of the fallen firefighters will be escorted home. The procession will leave Flagstaff at 2:00 p.m. and is expected to arrive in Show Low around 5:00 p.m.

Officials advise there may be traffic delays along the route. The best place for community members to gather and pay their respects will be along the Deuce of Clubs between Highway 77 and White Mountain Road.

Timber Mesa Fire and Medical District announced that its administration office will be closed during the procession as staff and firefighters honor their fallen colleagues.

The Arizona Department of Public Safety continues to investigate the cause of the collision.
